COVID-19 VACCINE

 

VACCINATION APPOINTMENTS

We are currently scheduling vaccination appointments using the Moderna vaccine which is a 2 dose series (spaced 4 weeks apart)

Due to the vaccine storage and use requirements, we must ensure that we utilize all available doses and appointments may need to be adjusted.

WHO IS ELIGIBLE FOR THE VACCINE?

Starting 4/15/21, anyone age 18+ is eligible for the Moderna vaccine.

Please note, only the Pfizer vaccine is approved for individuals age 16-18, the Moderna vaccine is only approved for individuals age 18+

You DO NOT need to be an established patient to receive the COVID-19 vaccine.

WHAT DO I NEED TO PROVIDE?

  • Please bring your insurance card. There is no fee for the patient. Face covering is required. 

  • 1st DOSE- Prior to your appointment, review the files below. Read through the vaccine information sheet. Print, fill out, and bring the "first shot" files.

  • If you are a new patient to our clinic, please print & fill out the consent form for the clinic. You do not need to fill this form if you are an established patient.

  • 2nd DOSE- Print, fill out, and bring the "second dose" files. Also bring your vaccination card to the appointment. 

  • POST-VACCINATION- You will need to remain on site for 15-30 minutes after receiving your vaccine. Please see the V-Safe flyer below

MODERNA VACCINE RECIPIENT INFORMATION SHEET

Read through the vaccine information sheet prior to your vaccination appointment.

FIRST DOSE

Download, print, and bring FILLED out form to your appointment. Single file includes both a vaccine consent form and COVID-19 screening form.

SECOND DOSE

Download, print, and bring FILLED out form to your appointment. File includes only the COVID-19 screening form. Make sure to bring in your vaccination card.

POST VACCINATION (V-SAFE)

V-safe is a smartphone-based tool that uses text messaging and web surveys to provide personalized health check-ins after you receive a COVID-19 vaccine. Through v-safe, you can quickly tell CDC if you have any side effects after getting a COVID-19 vaccine. Depending on your answers to the web surveys, someone from CDC may call to check on you and get more information. V-safe will also remind you to get your second COVID-19 vaccine dose if you need one
